Ø Be sure to register for the FALL CORE 2023 Program ONLY.
Ø After completing online application & payment, you will receive a confirmation email with Forms attached.

Ø Fill out the attached Forms (in your Confirmation email) and bring to one of the Form Drop off dates
listed below to complete your child’s and Mail the Forms in (AYSO, PO Box 404, Kaneohe HI 96744)
registration the upcoming Fall season.

Divisions 6U to 19U will receive a $20 discount when registering during those dates

A $5 discount will automatically be applied for each additional player after the first one.
All players must be registered under the same account.

Update 5/3/23: All Online Registrations from 6U - 19U are now on a Waitlisted Basis.
Complete your sign up and get on the Waitlist for openings. There
is no charge/fee to be on the Waitlist. You will be notified
via email if your player is accepted off the Waitlist.

*Registration fee does not include the National Player Fee of $20.00 and a $2.75 transaction fee
that will be added on to the registration fee upon checkout.
**Registrations after May 31, 2023 will incur a $10 Handling in addition to the fees listed above.

4U Players Receive: Uniform (Tee Shirt) • Basic Photo Package • Supplemental Accident Insurance
5U to 19U Players Receive: Full Uniform (jersey, shorts, socks) • Basic Photo Package • Supplemental Accident Insurance
Payment: Is ONLY online via credit card or debit card.
NO CASH OR CHECKS WILL BE ACCEPTED
AT OUR WALK-FORM DROP REGISTRATIONS.
What to bring to the Form Drop Registration: (To Mail)
1. One copy of your signed Registration and Concussion forms.
2. If your child is new to Kaneohe AYSO or did not participate in the Fall 2022 season, please bring a proof of age document.
(birth certificate, passport, or any government issued document). If you are doing a mail-in since you missed our in-person Form Drops,
please include a copy of the birth certificate (new players or did not play Fall 2022) or upload birth certificate to the player profile.)
IMPORTANT NOTE:
Your registration is not complete until you attend one of our Form Drop registration events and submit a signed hard copy of your child's registration and concussion forms and check in at our uniform station to be fitted for a uniform.
We will have a limited amount of laptops and printers for use to assist in registering your child. There may be a wait to use the computers.
Refund Policy*
For players that need to withdraw from the program, a parent or guardian must notify the Registrar in writing via email ([email protected]) that the player wishes to withdraw. If a refund is desired, the following dates must be adhered to:
A Refund Request Form must be completed and submitted to the Registrar in order to process the refund. Please allow 2-3 weeks
for a refund to be processed.
6U to 19U Fall Season (August to November)
On or before May 31 - Full refund*
From June 1 to July 31 - Partial refund of $15
On or after August 1 - No refund
4U and 5U Season (January to February)
On or before November 15 - Full refund*
From November 16 to December 31 - Partial refund of $15
On or after January 1 - No refund
*The refund does NOT include the non-refundable AYSO National Player Fee ($20.00) and the Sports Connect transaction fee ($2.75) which is not reimbursable.
